Take a vital role with Vancouver Coastal Health as an Older Adult Outreach Nurse (RN/RPN) in Vancouver, BC. This position focuses on improving care for older adults with significant mental health challenges.
As part of the Cambie Older Adult Mental Health team, you will engage with older adults who have psychiatric and concurrent disorders. Your responsibilities will encompass conducting assessments, planning treatments, and providing education both to clients and their families. Collaboration with healthcare professionals and community resources will be essential in ensuring comprehensive care and support.
Key Responsibilities:
• Engage in crisis intervention and care coordination
• Conduct psychogeriatric assessments to determine needs
• Facilitate group and individual counseling sessions
• Provide direct patient care in various settings
• Communicate with healthcare providers and family members
Requirements:
• Registered Nurse or Registered Psychiatric Nurse with BCCNM
• Minimum three years in related mental health services
• Valid driver's license required in BC
• Knowledge of geriatric psychiatry and nursing standards
• Demonstrated counseling and communication proficiency
