What You Will Do
Summary
When active substance use is a part of a patient's care needs, the right support can change everything. Help shape the way substance use care is delivered across the hospital, ensuring each patient and clinical team has the support they need.
PHC's Urban Health & Substance Use Program is accepting applications for L4 Addiction Medicine Nurse Liaisons to join their team. These roles support patients admitted into acute care l's Hospital who are followed by the Addiction Medicine Consult Team (AMCT) , providing point-of-care nursing education, substance use and overdose education, and care planning, working closely with physicians, other nurses, pharmacists, social workers, peers, and other partners to strengthen harm reduction-focused care across the hospital, as well as clinical support for the Overdose Prevention Site.

Your duties will include:

Provide clinical consultation at the bedside:  Be a hands-on resource for nursing and interdisciplinary teams, offering clinical support with assessments, medication administration, withdrawal management, complex behaviours and other substance use-related care needs.

Lead education and knowledge translation:  Provide point-of-care coaching and formal education on substance use, overdose prevention, withdrawal management and acute-care practices, helping staff build confidence and apply evidence-based approaches in practice.

Support complex care planning and coordination:  Partner with teams to develop and implement care plans and behavioural support plans, identify strategies for patients with complex needs, and help translate acute-specific policies and practices into day-to-day care.

Connect the hospital, AMCT and harm reduction services:  Triage and support AMCT referrals, collaborate closely with the interdisciplinary peer team, provide clinical support to the Overdose Prevention Site, and strengthen…
