Primary Care Intake Nurse Coastal Health

This role requires you to assess referrals and clients based on urgency and need. You will facilitate transitions for clients while enhancing communication in the interdisciplinary team approach. Your contributions will support VCH's mission of equitable healthcare and cultural safety.

Key Responsibilities:

• Assess client needs and initial screenings

• Coordinate transitions with interdisciplinary teams

• Document all client care and maintain statistical data

• Initiate interdisciplinary conferences for complex cases

• Communicate capacity for new patient referrals

Requirements:

• Active nurse registration with BCCNM

• Minimum of two years in related clinical fields

• In-depth knowledge of nursing principles and ethics

• Experience with trauma-informed care techniques

• Strong interpersonal and teamwork capabilities
