Take on the role of Care Management Transition Leader with Vancouver Coastal Health at Vancouver General Hospital. Focus on effective patient care transitions within a collaborative framework.

As a key member of the Care Management Lead Therapy Ambulatory Team, you'll assess patients’ needs and develop sustainable discharge plans. This leadership position requires engaging with an acute care interprofessional team to facilitate timely transitions, applying your nursing expertise to optimize patient outcomes.

Your role will also involve implementing best practices to enhance overall care quality.

Key Responsibilities:

• Manage patient care transitions effectively

Requirements:

• RN with the British Columbia College of Nurses

• 3 years’ clinical nursing experience

• 1 year of supervisory or leadership experience
