Elevate community care as a Mental Health and Substance Use Outreach Nurse with Vancouver Coastal Health. Collaborate with an interdisciplinary team while providing essential triage and outreach services.
In this role, you will become an integral member of the Assertive Community Treatment team. Focus on delivering flexible care for individuals facing mental illness and substance use issues. Utilize your knowledge in crisis intervention, assessments, and treatment planning to foster positive client outcomes.
Key Responsibilities:
• Conduct mental and physical status assessments
• Provide crisis intervention and assess high-risk situations
• Collaborate on client care plans with the interdisciplinary team
• Facilitate client transitions between care services
• Assist clients with medical appointments and support needs
Requirements:
• Registered Nurse or Registered Psychiatric Nurse in BC
• Two years of experience in mental health and substance use
• Valid BC Driver’s License for business purposes
• Knowledge of psychopharmacology and treatment principles
• Strong communication skills with interdisciplinary teams
