Position: Patient Navigator for Musqueam Health Services
Empower Musqueam community members as a Patient Navigator by facilitating their access to healthcare and essential resources. Advocate for health outcomes and promote cultural understanding.

As a Patient Navigator, you will report to the Primary Clinic Practice Manager, playing a crucial role in connecting patients with services, managing their appointments, and fostering strong communication between individuals and healthcare providers. This role demands a compassionate approach to care in acknowledgment of the impact of systemic racism.

Key Responsibilities:

• Coordinate and manage patient schedules and appointments

• Facilitate transportation arrangements for healthcare access

• Educate patients on their rights and treatment options

• Provide support for medical form submissions

• Advocate for health resources within the community

Requirements:

• Completion of relevant post-secondary education

• Minimum of 2 years' experience in a healthcare setting

• Knowledge of BC healthcare systems

• Strong organizational and prioritization skills

• Access to a vehicle and valid driver’s license
