Are you passionate about walking alongside women on their journey to recovery—bringing hope, healing, and Christ’s love every step of the way? The Case Manager is responsible for the women in the Joanna Home Stabilization & Recovery program from intake to discharge. This position makes the first contact with those who have been referred to Joanna Home and conduct interviews; provide tours;
introduce new participants to the Program; update ETO; assist with intake/discharge tasks and orientation. You will create and maintain networks with external partnering organizations, and will regularly attend meetings with other external service providers. You work with participants individually in order to increase the likelihood of success for recovery and/or independent living while ensuring adherence to program guidelines and policies.
The Case Manager performs client assessments to gather information and identify areas of concern, helps participants develop plans for next steps, connects them with appropriate resources and may even advocate on their behalf with government agencies or external service providers. The Case Manager takes a faith-based approach, which includes ministering to these women, forming relationships, loving them, praying, declaring the good news of Jesus Christ and making disciples, as well as assisting with practical needs such as referrals for recovery, program involvement, and housing.
In addition, this position may plan and lead a devotion or group sessions and teach classes, and may be required to give direction to junior staff from time-to-time. This is a permanent
, part-time
position (22.5 hours). The starting range for this position is $28.96 to $31.64 per hour.

Duties And Responsibilities Case Management (40%)
- Assist participants in establishing goals, monitoring progress, and preparing a case plan for their next steps (housing, employment, addiction services, life skills, health, education or other case specific needs) and refer them to the appropriate community resources for achieving these goals, using various tools, programs, and training to facilitate.
- Document all relevant case notes and maintain accurate case files and records for future reference.
- Analyze, screen, interview, and process all intake applicants.
- First point of contact upon arrival. Complete paperwork, introduce and orient to the new building and complete/assist bed bug protocol, key system etc.
- Assists each participant with discharge, including research, referrals, and administrative support.
- Provide care, direct consultation and spiritual guidance to participants who reside at the Sanctuary, which includes providing appropriate support for their emotional needs and practical requests as needed. Build strong relationships with participants and encourage them to explore Christianity and recovery as per their goals.
