Become a key member of a collaborative interdisciplinary team as a Social Worker in a long-term care environment. Provide vital support to residents and families, ensuring compassionate and personalized care aligned with community values.

This role entails offering professional social work services facility-wide, including pre-admission assessments and continuous support throughout the settling-in process. The Social Worker will facilitate educational and support groups, addressing any social, emotional, or economic challenges faced by residents and their families. Experience in geriatric care will enhance the impact of your work in this community-focused environment.

Key Responsibilities:

• Coordinate social work services and overall care strategies

• Conduct pre-admission interviews and provide information

• Offer adjustment counselling to residents and families

• Manage educational support groups and community outreach

• Document in resident health records according to guidelines

Requirements:

• Master’s or Bachelor’s in Social Work from accredited institution

• Registration with BC College of Social Workers

• Minimum two years experience in Long-Term Care or Geriatrics

• Strong communication and interpersonal skills

• Knowledge of Jewish Values and Traditions an asset
