Empower residents at Grace Mansion as a Transitional Housing Caseworker. Support personal development and facilitate life skills while fostering a nurturing community environment.
As a Transitional Housing Caseworker, you will work closely with residents, focusing on life skills and social activities.
Your role involves conducting orientations, assessing needs, and connecting clients with essential services. Additionally, you'll provide hands-on coaching to help residents achieve their personal goals within a supportive atmosphere.
Key Responsibilities:
• Conduct client orientations and review facility rules
• Develop relations with service providers for case management
• Provide hands-on training and coaching in life skills
• Conduct initial assessments within 10 days of arrival
• Prepare exit plans for eventual client transition
Requirements:
• Minimum 2 years’ experience with homeless individuals
• Post-secondary degree in relevant field
• Certifications in First Aid/CPR and conflict intervention
• Strong communication and organizational skills
• Criminal Record Review Act clearance required
