Join Hollyburn Family Services as a Home Share Coordinator, where you will play a vital role in person-centered community support. This full-time position prioritizes relationship-building and program coordination, utilizing your organizational skills.

This leadership role reports to the Senior Program Manager – Outreach and requires oversight of Home Share and Community Inclusion programs. You will assist Home Share providers and ensure quality service delivery aligned with CLBC standards. Your background in social services will drive effective case management and development.

Key Responsibilities:

• Coordinate Home Share and Community Inclusion services

• Recruit and support Home Share providers

• Conduct client and provider meetings and visits

• Monitor program quality and service outcomes

• Supervise Community Inclusion Workers and support staff

Requirements:

• Bachelor’s degree in Social Services or equivalent experience

• Minimum 5 years in social services

• At least 1 year of supervisory experience

• Strong case management and critical thinking skills

• Proficiency in Microsoft Office Suite
