Key Responsibilities:

• Conduct interviews to gather client and family information

• Guide clients to appropriate external resources

• Assist with benefits, housing, and community service navigation

• Keep abreast of available services and applications

• Collaborate with social workers during patient admissions

Requirements:

• Relevant education in Social Services or equivalent

• Effective communication and interpersonal skills

• Ability to manage and organize tasks efficiently
