ROLE PURPOSE
This position will provide intensive supports and services to eligible individuals through the Homelessness Prevention Program (HPP), funded by BC Housing, facilitating access to supports services that will assist in meeting their housing needs.
STATUS
- This is a temporary position until May 2027, or until the return of incumbent.
- Unionized. This position is open to applicants of all genders.
HOURS (20 hours per week)
- Tuesday & Wednesday, 0900 - 1700, and Thursday, 0900 – 1300.

REPORTING AND WORKING RELATIONSHIPS
- Reports and is accountable to the Program Manager or their designate
- Participates as a member of the Connective Services Team to ensure quality and continuity of services
- Liaises with staff and volunteers of community and government agencies as required, to establish a network of housing professionals addressing the challenges and barriers experienced by our service users.
- Liaises with the staff of Connective and other community partners as necessary to ensure timely communication and an integrated team approach
- Works in collaboration with other community supports to ensure that participant needs are effectively met
KEY ACCOUNTABILITIES
- Receives and reviews referrals from Connective’s Community Services Office and community partners to determine eligibility and suitability for the program
- Provides outreach, case management, advocacy, and other needed services to participants in any environment including streets, shelters, hospitals, institutions, apartments, and the drop-in office
- Works with each individual to create a mutually agreed intervention and support plan that has measurable short and long-term goals
- Ensures issues of daily living, including but not limited to housing, basic needs, hygiene, leisure, employment, and education are addressed in this plan
- Supports and provides services for individuals who are homeless, or at risk of homelessness, and have lived experiences of substance use, mental health, involvement in the criminal justice system, intimate partner violence, and trauma
- Provides a wide range of interventions to enable each service user to address and accomplish the established goals
- Interacts with other professionals, family members and organizations to ensure services are in the service user’s interest and supportive of the long-term goals
- Maintains written and computerized records, compiles reports and completes other program documentation (including case notes, statistics, letters, available file information, etc.) in accordance with the policy and procedures manuals of the society and in accordance with funder requirements
- Initiates, coordinates and monitors referrals to community services, and advocates for service users’ participation in them
- Creates, builds, and fosters partnerships with housing providers and landlords
- Educates, informs and advocates for participants regarding benefits and entitlements (social assistance, rental subsidies, employment supports, low income services i.e. food bank and food stamps)
