This part-time position involves engaging with women facing homelessness and health issues through direct community outreach. You'll provide essential social support while working in a culturally safe manner. The position fosters collaboration with various health and social services to enhance client outcomes.
Key Responsibilities:
• Attend outreach visits and serve as the primary contact
• Conduct triage and assessments to prioritize client needs
• Collaborate with clients to create and implement case plans
• Connect clients with available resources and services
• Ensure accurate record-keeping in the electronic system
Requirements:
• Bachelor’s or Master’s Degree in Social Work
• Current registration with the BC College of Social Workers
• Experience in outreach or social service roles
• Proven advocacy and navigation skills
• Commitment to culturally grounded and trauma-informed care
Help women access vital services and improve their well-being at VAHS.
