Key Responsibilities:

• Conduct social assessments for clients with complex needs

Requirements:

• Master’s Degree in Social Work is essential

• Two years’ experience with geriatric and chronic illness cases

• Registered with the British Columbia College of Social Workers

• Valid BC Driver’s License required for travel
