Community Social Worker Coastal Health

Join Vancouver Coastal Health as a Community Social Worker at Three Bridges Community Health Centre. Support individuals with chronic health issues and mental health challenges through expert social work services.

In this role, you will conduct psychosocial assessments, establish therapeutic relationships, and develop care plans tailored to client needs. Collaborating with a multidisciplinary team, you will engage in consultation and ongoing evaluation to ensure high-quality care and community well-being.

Key Responsibilities:

• Assess client situations and design effective interventions

• Maintain meaningful therapeutic relationships with clients

• Facilitate team planning and community engagement

• Enhance service quality through evaluation and improvements

• Act as a Designated Responder under applicable legislation

Requirements:

• Current registration with the BC College of Social Workers

• Master’s degree in Social Work required

• Two years of experience in mental health settings

• Must possess a valid BC Driver’s License

• Strong case management and communication skills
