Vancouver Coastal Health is seeking a dedicated individual with a Master’s Degree in Social Work and three years of experience in case management. As a part of a multidisciplinary team, you will conduct psychosocial assessments, provide counselling services, and create care plans tailored to client needs. This position emphasizes collaboration while adhering to principles of equity and Indigenous Cultural Safety.
Key Responsibilities:
• Conduct functional and psychosocial assessments to identify client needs
• Provide individual and group counselling for clients and families
• Maintain comprehensive client records and documentation
• Develop and implement care plans with multidisciplinary team members
• Coordinate transition and continuity of care for clients
Requirements:
• Master’s Degree in Social Work with BC registration
• Minimum three years’ experience working with geriatric clients
• Valid BC Driver’s License for local travel
• Knowledge of intensive case management principles
• Ability to apply conflict resolution and mediation skills
