In this Social Worker role with Vancouver Coastal Health, you'll integrate into the Assertive Community Treatment team model, providing continuous support for those facing serious mental health and substance use issues. Your focus will be on employing a client-centered and trauma-informed approach to ensure clients receive necessary interventions and support for recovery and wellness.
Key Responsibilities:
• Deliver comprehensive assessments for individuals in need
• Collaborate with clients and teams for service continuity
• Provide consultative support on complex social issues
• Document and maintain accurate client records
• Support clients in accessing health and social services
Requirements:
• Registration with British Columbia College of Social Workers
• Master’s degree of Social Work
• Minimum two years’ experience in healthcare
• Proficiency in conflict resolution and teamwork
• Understanding of recovery principles and resources
