In this role, you will work closely with residents, developing care plans, providing emotional support, and facilitating day-to-day activities. Commitment to clients with developmental disabilities and mental health challenges is essential, alongside collaboration with a multidisciplinary team.
Key Responsibilities:
• Conduct client interviews for care plan preparation
• Provide input on care plans to achieve resident objectives
• Monitor resident progress and discuss future interventions
• Assist in daily living activities and emotional support
• Maintain detailed client files and documentation
Requirements:
• Diploma in social work or community social services (asset)
• 1 year experience with adults facing developmental/intellectual disabilities
• Strong crisis management skills
• BC Class 5 driver’s license required
• Current First Aid Certificate and Criminal Records Check
