Case Manager for Community Services

Empower adults with disabilities and the elderly as a Community Services Case Manager at the Area Agency on Aging & Disabilities of SW WA. Enjoy a family-friendly M-F schedule with no holidays or on-call duties.

This role focuses on assessing client needs and developing individualized service plans to promote independence. You will work closely with older adults, caregivers, and individuals with disabilities, navigating resources in Clark, Cowlitz, Wahkiakum, Klickitat, and Skamania Counties. Your advocacy ensures clients receive necessary in-home and community services.

Key Responsibilities:

• Conduct client assessments using various standardized tools

• Develop and administer personalized service plans

• Authorize community-based support services

• Promote equity and access to important programs

• Facilitate transitions between care settings

Requirements:

• Master’s degree in behavioral or health science plus 1 year experience

• Valid driver's license and reliable transportation

• Strong knowledge of community resources

• Ability to work independently in clients' homes

• Experience in service planning and implementation

Support client well-being and independence through effective case management.
