You will work closely with clients to address their comprehensive needs, creating a foundation of trust and engagement. This role involves flexible scheduling, engaging in community inclusion programs, and facilitating connections to essential services. You’ll play a critical part in fostering independence and well-being.
Key Responsibilities:
• Participate in care plan development collaboratively
• Model positive behaviors and provide support
• Identify and connect clients with community resources
• Collaborate with team members for effective plans
• Oversee and enhance the facility's operational quality
Requirements:
• A valid BC class 5 driver's license is mandatory
• Experience with complex, high-risk individuals
• Relevant diploma in social work or related field
• Demonstrated crisis management skills
• Emergency First Aid Certificate required
