Faith- Case Manager

Position: Faith-Based Case Manager
Support women on their journey to independence as a Case Manager with UGM in Vancouver. This full-time role merges practical care with spiritual support within the Sanctuary Stabilization program.

As a Case Manager, you will oversee the intake process, create personalized recovery plans, and mentor women towards achieving their goals.

Your role involves building relationships, providing spiritual care, and advocating for program participants with community partners. A genuine commitment to service is essential in this challenging yet rewarding position.

Key Responsibilities:

• Orient new participants and complete intake paperwork

• Develop and implement case plans for recovery

• Monitor progress and document important case information

• Lead classes and group therapy sessions

• Engage with community resources and external partners

Requirements:

• Minimum 2-3 years' related experience

• Understanding of addiction and poverty issues

• Ability to communicate clearly and effectively

• Strong advocacy and problem-solving skills
