As a Case Manager, you will play a pivotal role in the Sanctuary Stabilization program, managing intake processes and facilitating individual care plans. Your responsibilities will include guiding participants toward community resources, providing spiritual encouragement, and advocating for their needs. Building relationships with external organizations will enhance the support network available to participants.
Key Responsibilities:
• Conduct interviews and process intake applications
• Create and track progress on case plans
• Offer direct spiritual care and support
• Conduct group sessions and classes
• Maintain accurate case documentation and records
Requirements:
• Relevant post-secondary education required
• 2-3 years of experience in a related role
• Strong skills in crisis resolution and participant advocacy
• Knowledge of poverty and addiction issues
• Alignment with UGM’s mission and values
