Interim Assistant Nurse Manager, Care Management,
for a W2 Contract position based in Salem, Oregon and Vancouver, Washington.

Schedule: Full time, Monday through Friday, 8am-5pm

Assignment Duration: 13 Weeks with high likelihood of extending

Compensation:

This position offers a daily rate of $685 to $700 which equates to $3,425 to $3,500 per week (based on a 5‑day workweek)

Per Diems:
Lodging: $155 per day and Meals & Incidentals: $86 per day

Per diems are paid daily, 7 days a week, totaling $1,687 per week

Auto Stipend: $25 per day

Stipend is paid daily, 7 days a week, totaling $175 per week
Total Weekly Compensation (Daily Rate + Per Diems + Stipend): Between $5,287 to $5,362 per week

Benefits: Paid sick leave, Paid holidays, Co‑paid medical, dental, and vision insurance, Cafeteria plan access and 401(k) retirement plan.

Summary:

The Assistant Nurse Manager, Care Management provides operational and clinical leadership for Care Management services across multiple sites. The role will begin with oversight of the Care Management department in Salem, then transition in late January/early February to assume leadership of the Care Management department in Vancouver, while maintaining continued oversight of Care Management operations in Salem. Responsibilities include planning, organizing, directing, and evaluating care management functions to ensure high‑quality, cost‑effective, and patient‑centered services.

The Nurse Manager collaborates closely with interdisciplinary teams and organizational leadership to support safe transitions of care, regulatory readiness, utilization management, and ongoing performance improvement across the continuum.

Essential Functions
  • Provides daily operational and clinical leadership for Care Management services in Salem, later expanding to include oversight of the Vancouver Care Management department.
  • Oversees coordination, planning, and implementation of care management processes, ensuring efficient transitions of care, timely discharge planning, and effective interdisciplinary communication.
  • Ensures compliance with federal, state, and local regulations, organizational policies, and accreditation standards relevant to Care Management operations.
  • Develops and implements departmental goals, quality initiatives, and standards to support utilization management and improve patient outcomes.
  • Supervises, mentors, and evaluates Care Management staff, promoting professional development and consistent practice across both locations.
  • Partners with leaders and stakeholders to identify workflow improvements, implement innovative care coordination strategies, and enhance continuity of services across sites.
  • Maintains continuous regulatory readiness, oversees audit preparedness, and ensures accurate documentation and reporting for compliance.
  • Manages departmental resources, including staffing, budgeting, and cost‑control initiatives, while maintaining high‑quality, patient‑centered service delivery.
  • Serves as a resource for escalated issues involving patient flow, discharge barriers, utilization, risk management, and patient experience.
  • Utilizes data, metrics, and evidence‑based practices to guide decision‑making and strengthen care management operations in both Salem and Vancouver.
Qualifications
  • Bachelor of Science in Nursing (BSN) or BA in a health care–related field required (experience in lieu of degree will be considered. Master’s degree preferred. Graduate of an accredited school of nursing.
  • Current Oregon and Washington RN license required.
  • BLS and ACLS required.
  • Clinical nursing experience relevant to Care Management (around 3–4 years), including at least 2 years of leadership experience required.
  • Demonstrated knowledge of care coordination, utilization management, discharge planning, regulatory standards, and accreditation requirements required.
  • Strong leadership, communication, interpersonal, and collaboration skills with the ability to support operations across multiple sites required.
