Licensed Practical Nurse – Home Health
Location: Vancouver, WA
Schedule: Full Time, 1.0 FTE, Day shift
Salary Range: $28.36 – $42.03 per hour (dependent on education, training, experience, and collective bargaining agreement terms)

, we are dedicated to bringing compassionate, personalized care into the homes of our patients. We are currently seeking a Licensed Practical Nurse (LPN) – Home Health to join our Vancouver team. This role is an excellent opportunity for an LPN who values teamwork, professional growth, and making a meaningful difference in the lives of patients and families.
- Provide direct nursing care under the supervision of an RN, following physician orders and established care plans.
- Administer prescribed medications and treatments, and monitor patient responses.
- Document all care accurately in accordance with department standards.
- Assist in the development and evaluation of patient-specific care plans.
- Perform wound care, dressing changes, vital sign monitoring, patient education, and other direct care duties.
- Collaborate with the healthcare team to ensure safe, timely, and patient-centered care.
- Support daily operations by transporting supplies, equipment, and specimens as needed.
- Maintain professional effectiveness through ongoing education and development.
- Graduate of an accredited State Board of Nursing LPN program.
- Current Washington State LPN license.
- Current Basic Life Support (BLS) certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- Minimum 1 year of LPN clinical experience preferred.
- Knowledge of age‑specific principles of growth, development, and life stages.
