Licensed Practical Nurse – Home Health

Location: Vancouver, WA

Schedule: Full Time, 1.0 FTE, Day shift

Salary Range: $28.36 – $42.03 per hour (dependent on education, training, experience, and collective bargaining agreement terms)

This role is an excellent opportunity for an LPN who values teamwork, professional growth, and making a meaningful difference in the lives of patients and families.

• Provide direct nursing care under the supervision of an RN, following physician orders and established care plans.
• Administer prescribed medications and treatments, and monitor patient responses.
• Document all care accurately in accordance with department standards.
• Assist in the development and evaluation of patient-specific care plans.
• Perform wound care, dressing changes, vital sign monitoring, patient education, and other direct care duties.
• Collaborate with the healthcare team to ensure safe, timely, and patient-centered care.
• Support daily operations by transporting supplies, equipment, and specimens as needed.
• Maintain professional effectiveness through ongoing education and development.

• Graduate of an accredited State Board of Nursing LPN program.
• Current Washington State LPN license.
• Current Basic Life Support (BLS) certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Minimum 1 year of LPN clinical experience preferred.
• Knowledge of age‑specific principles of growth, development, and life stages.
