Enhance patient outcomes as a Registered Practical Nurse in our Community Visiting Nursing Program. Provide essential nursing care to diverse patients primarily in the Woodbridge/Maple vicinity across various shifts.
In this rewarding role, you'll partner with patients, families, and other healthcare professionals to develop tailored nursing care plans. Your contributions will help evaluate care effectiveness and ensure patient advocacy, safety, and comfort in their homes. Key Responsibilities:
• Create plans of care with patients and caregivers
• Deliver nursing services according to care plans
• Assess treatment effectiveness and make necessary adjustments
• Safeguard patient rights as an advocate
• Educate peers on nursing practices and protocols
Requirements:
• Current RPN certification with the College of Nurses of Ontario
• One year of experience in community nursing
• Strong interpersonal and communication skills
• Current CPR and clear VSS required
• Valid driver's license and dependable vehicle
