The Thornhill Community Health Centre (CHC) is an interdisciplinary primary care and community health organization committed to providing accessible, inclusive, and proactive care for individuals and communities facing barriers to care.

Thornhill CHC incorporates accessibility-focused, disability-informed, and equity-centred approaches to care, with a particular focus on supporting individuals with intellectual and developmental disabilities (IDD), neurodiverse individuals, seniors, caregivers, and other underserved populations.

The CHC works collaboratively with community partners, healthcare providers, and interdisciplinary teams to improve access to comprehensive, person-centred care and promote health equity across the community.

About the position
The Registered Nurse (RN) or Registered Practical Nurse (RPN) provides comprehensive, client-centred primary health care services as part of an interdisciplinary team. Care is delivered to individuals and families who may experience barriers to achieving optimal health and wellness, using trauma-informed, culturally responsive, and disability-informed approaches.

The RN/RPN practices within their respective scope in accordance with the standards of the College of Nurses of Ontario (CNO), organizational policies, and the Community Health Centre (CHC) model of care.

The complexity of clinical responsibilities will align with the individual’s designation, experience, and competencies. While Registered Nurses typically manage more complex clinical situations, experienced Registered Practical Nurses with strong backgrounds in community health, primary care, or specialty clinic settings are encouraged to apply.

Hours of Work

This position follows a CHC team-based primary care model with scheduled clinical hours as follows:

  • Monday–Wednesday: 9:00 a.m. – 5:00 p.m.
  • Thursday: 8:00 a.m. – 8:00 p.m.
  • Friday: 9:00 a.m. – 1:00 p.m.
  • Sunday: 9:00 a.m. – 1:00 p.m. (approximately once per month)

Total weekly hours: 37.5 hours (CHC standard). A 30-minute unpaid lunch break is scheduled Monday to Friday.

During the initial operational phase, these hours will apply as outlined above. Once the clinic is fully operational, hours may be adjusted in alignment with service needs and interdisciplinary team-based scheduling.

This role also includes periodic participation in shared after-hours on-call coverage with physicians and nurse practitioners (typically a few days every few months).

Salary range: $60,000 – $77,000, based on experience and credentials (RPN vs. RN).

Key Responsibilities

1. Direct Care Responsibilities

  • Assist in the delivery of primary care services and promote healthy lifestyle choices for individuals, families, and the broader community.
  • Provide holistic nursing care and act as an entry point into the broader health and social service system.
  • Conduct nursing assessments, health education, counselling, triage, history taking, and follow-up care within RN scope of practice.
  • Support chronic disease management, preventive care, screening, immunizations, and health promotion activities.
  • Support individuals with intellectual and developmental disabilities (IDD), neurodiverse individuals, seniors, caregivers, and clients experiencing barriers to care.
  • Coordinate care internally and externally with specialists, hospitals, community agencies, and interdisciplinary providers.
  • Participate in treatment planning, health teaching, counselling, and care coordination activities.
  • Provide telephone triage and nursing advice as appropriate.
