As a key member of the interdisciplinary primary care team at Thornhill CHC, you will provide client-centered healthcare across the lifespan. Your responsibilities will involve patient assessment, diagnosis, treatment, and ongoing management of both acute and chronic conditions. You will collaborate with fellow healthcare professionals to ensure integrated and accessible care for underserved communities.
Key Responsibilities:
• Provide comprehensive primary care services within your NP scope.
• Conduct thorough health assessments for clients.
• Diagnose and manage a variety of health conditions.
• Order and interpret necessary diagnostic tests.
• Collaborate with mental health professionals for integrated care.
Requirements:
• Registered Nurse in Extended Class with Ontario College.
• 2–3 years of clinical experience as NP or RN.
• Experience with vulnerable populations and complex health needs.
• Proficiency in Electronic Medical Records.
• Commitment to person-centered care principles.
Deliver meaningful healthcare solutions and enhance patient outcomes as a Nurse Practitioner at Thornhill Community Health Centre.
