Bayshore Health Care is seeking a Registered Social Worker to provide community-based social work services in a home-care setting in Thornhill, ON. The role is casual and Independent Contractor, with a pay rate of $70-$100 per hour.
Responsibilities include collaborating with clients, families, and other professionals, maintaining caseloads, assessing psychosocial needs, and coordinating referrals to community resources while upholding PFCC principles.
