Licensed Social Worker

Position: Licensed Social Worker at Mackenzie Health
Become a key part of Mackenzie Health as a Licensed Social Worker. Provide essential psychosocial assessments, discharge planning, and counseling within a collaborative interprofessional team.

As a Social Worker in the Continuing Care Program, you will engage with patients and families to enhance psychosocial health and coordinate care plans. Your work will encompass providing education, advocating for patient needs, and accessing community resources. This role requires expertise in psychosocial services, strong decision-making skills, and the ability to manage complex cases effectively.

Key Responsibilities:

• Conduct psychosocial assessments for patients and families

• Advocate for patient care decisions and resources

• Develop and implement discharge plans with a holistic approach

• Collaborate with interdisciplinary teams for effective care

• Lead bereavement care within the Continuing Care Program

Requirements:

• Master’s degree in Social Work from an accredited university

• Current registration with the Ontario College of Social Workers

• 2-3 years of experience in a hospital environment preferred

• Strong interpersonal and communication skills

• Ability to manage and prioritize multiple complex cases
