Een zorgorganisatie in Veendam zoekt een Trajectbegeleider voor 28 uur per week. Je begeleidt cliënten met langdurige problemen en stimuleert herstel en zelfredzaamheid. Als je MBO niveau 4 in Maatschappelijke Zorg hebt en ervaring hebt met psychosociale problematiek, ben je welkom om te solliciteren.
