Agathos hulpverlening zoekt een intensief ambulant jeugdhulpverlener die gezinnen, kinderen en jongeren ondersteunt in complexe opvoedings- en gedragsproblematiek. Je werkt volgens oplossingsgericht werken en versterkt het netwerk rond de cliënt.
Je bent veel bij de cliënt thuis en signaleert proactief wat nodig is. Je onderhoudt nauwe samenwerkingen met gemeenten en andere zorgprofessionals en bewaakt samen met het team de regie en veiligheid van cliënten.
