Brabant Zorg zoekt een gediplomeerde diabetesverpleegkundige met voorschrijfbevoegdheid voor de regio Oss en omgeving. Je beheert diabeteszorg bij kwetsbare ouderen en revalidatiepatiënten, werkt zelfstandig en informeert andere disciplines zoals diëtisten en artsen.
Je ziet toe op behandeltrajecten, begeleidt collega’s en ontwikkelt protocollen, met aandacht voor kwaliteit en leefklantcentra. Je rapporteert aan zorginstellingen en houdt rekening met poliklinische en intramurale zorg.
