ThuiZ Velserbroek zoekt een persoonlijke begeleider om bewoners te ondersteunen in een huiselijk woonzorgtraject. Je bent het eerste aanspreekpunt voor ouders en naasten en draagt zorg voor het actuele begeleidingsplan.
Je leidt de dagelijkse begeleiding, stelt schema’s en afspraken op en bespreekt deze in teamvergaderingen. Er is 24 uur per dag een begeleider aanwezig, met dienststart om 15:30/17:00 en doorlooptijden tot de volgende ochtend.
