Position: Hospital Access Navigator (RN) - Weekends

Tidewell Hospice, a member of Empath Health is currently seeking a Hospital Access Navigator (RN) to join our team. This role is a weekend position (Saturday - Monday) and primarily covers Sarasota Memorial Hospital and Venice Hospital.

The Hospital Access Navigator is an integral component of the leadership access team within the Admission Department and has oversight of the day to day work completed by the Access Team staff members. This position assumes responsibility for conveying the Mission/Vision to the hospital served by Empath Health. For all service lines, the Hospital Access Navigator is directly responsible for compliance with governing state and federal regulations and standards of care guidelines of outside accreditation agencies.

This individual has the primary responsibility and authority for maintaining quality of patient care and overall results in accordance with the organization’s objectives and policies through planning, coordinating, implementing, and evaluating the services provided to patients and families.

JOB DUTIES/RESPONSIBILITIES Thought Leadership
  • Executes organizational strategic plan and Empath goals & objectives.
  • Aligns access team activities with organizational mission, vision, goals and strategies.
  • Leads access team in development and successful integration of cascading goals.
  • Identifies efforts or actions that will have the greatest strategic impact.
  • Responsible for access team conversion rate and KPIs within hospitals assigned.
  • Manages the access team’s productivity along with assumes responsibility for actively contributing in process improvement efforts.
  • Ensures patient satisfaction and responsibility for access team’s CECs and customer experiences.
  • Demonstrates solid knowledge of Medicare, Medicaid, insurance benefits with all Empath Health programs.
  • Business Acumen
  • Ensures access team decision making and care reflects knowledge of and compliance with agency policies & procedures and governing regulatory bodies.
  • Identifies and integrates diverse ideas and perspectives in order to generate effective solutions.
  • Demonstrates an understanding of key financial indicators and metrics necessary to measure and improve performance.
  • Operationalizes financial/metric analysis to appropriately guide business decisions promoting financial success.
  • Compares departmental performance to industry benchmarks.
  • Develops action plans and processes to improve sub‑optimal quality and identify gaps or opportunities to improve levels of service to customers, according to agency policies.
  • Partners with key hospital partners to strengthen relationships and establish Empath as a preferred provider.
  • Engages and visits hospitals in collaboration with Business Development and Programs.
