Duties Of Position: Registered Nurse provides nursing care in accordance with the client’s Plan of Care, performing comprehensive health and psychosocial evaluations, monitoring condition, promoting sound health and preventive practices, coordinating services, educational/training activities, and direct nursing.

• Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring client condition, promoting sound preventive practices, coordinating services and educational and training activities.
• Evaluates effectiveness of nursing services to the patient and family.
• Prepares and presents patient’s record to the Clinical Record Review Committee, as indicated.
• Consults with attending physician concerning alterations of patient care plans, checks with appropriate supervisor, makes changes as appropriate.
• Submits daily tally of patient care visits.
• Participates in case conferences, discusses problems concerning patients and how best to handle them with supervisor.
• Discusses need for involvement of other health team members (home health aide, PT, OT, speech therapy, medical social worker, etc.) with supervisor.
• Obtains orders for paraprofessional services and submits referrals to appropriate personnel.
• Participates in patient discharge planning process.
• Cooperates with other agencies providing nursing or related services for continuity of care and comprehensive care plan implementation.
• Participates in staff development meetings.
• Continually strives to improve nursing care skills by attending in‑service education, formal education, workshops, conferences, active participation in professional and related organizations and individual research and reading.
• Participates in development and periodic revision of physician’s plan of treatment and processes change orders as needed.
• Enters clinical notes into EMR within required hours of scheduled visit (24 hrs for non‑Oasis, 48 hrs for Oasis) and prepares progress notes and other clinical record forms outlining services rendered.
• Maintains on‑going knowledge of current drug therapy.
• Substitutes for other nurses as requested by Director of Nursing.
• Complies with agency dress code policy.
• Maintains efficient flow of agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests.
• Maintains current TB testing (refer to Health Screening Policy).

Frequent telephone communication. May be required to bend, stoop, reach and move client weight up to 250 lb and lift/carry up to 30 lb. Hearing, vision, and physical dexterity must be sufficient to perform physical assessment, demonstrate client care, and assess a client’s home that may not be wheelchair accessible. Must communicate well orally and in writing. Must have driver’s license and be willing and able to drive to client’s residence.

• Graduate from an accredited School of Nursing.
• Licensed in the State as a Registered Nurse.
• One or more years in home health agency or in a hospital setting preferred.
• Knowledge of Medicare guidelines preferred.
• Preferred working knowledge of home health care and the principles and techniques of professional nursing and required documentation.
• Skillful in organization and time management principles and management processes.
• Contribute to quality of care through constructive communication with nursing managers and staff.
