VIGO Groep zoekt een zorgcoördinator voor Korsakov Wonen in Venray. Je regisseert het zorgproces binnen het team, organiseert dagelijkse zorg en verdeelt taken onder medewerkers. Je bewaakt de kwaliteit en zorgt voor duidelijke communicatie.
Je werkt samen met regiebehandelaar, expertiseleider en teamleider. Je stelt jaarlijks een jaarplan op en evalueert afspraken. Een HBO-verpleegkunde met BIG-registratie is vereist en je bent bereid om 32–36 uur te werken.
