What You'll Do
- Deliver patient-centered care that brings warmth and dignity to every interaction
- Administer medications and treatments safely and accurately
- Monitor residents' health and document changes with precision
- Collaborate with a multidisciplinary team to create and update individualized care plans
- Maintain a clean, safe environment that promotes comfort and wellness
- Communicate with residents, families, and team members to support whole-person care
- Active LPN license in the State of Ohio
- Current CPR certification
- Experience in skilled nursing, assisted living, or long-term care preferred
- Strong clinical skills and attention to detail
