** We are seeking a dedicated and compassionate
** Nurse Practitioner or Physician Assistant to
** join our team at Helen Porter, a 120-bed facility specializing in long-term care and rehabilitation.

This role offers a consistent part-time schedule with opportunities for additional shifts and future growth to full-time based on patient census and care needs.##

* Key Responsibilities:

*** Provide high-quality care to a diverse patient population, primarily long-term care residents with some rehabilitation patients.
* Collaborate with a multidisciplinary team including physicians, nursing staff, and administrative leaders.
* Conduct patient assessments, develop care plans, and manage ongoing treatment.
* Document care using
** Epic EMR** (training provided).
* Maintain compliance with facility protocols and regulatory standards.## ##
** Schedule & Coverage:**
* ** Standard

Schedule:

** 2 set days per week (Wednesday and Friday preferred).
* ** Shift Length:
** 10-8-hour shifts
* ** Coverage Area:
** 120-bed facility (primarily long-term care).
* ** Team Structure:
** 2 physicians + 1 full-time APP (typically 2 providers onsite daily).## ##

* Qualifications:

*** Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Vermont.
* Experience in long-term care, geriatrics, or rehabilitation preferred.
* Strong communication and collaboration skills.
* Ability to work independently and as part of a team.
