Nurse Practitioner or Physician Assistant- Helen Porter

We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Helen Porter, a 100‑bed facility specializing in long‑term care and sub‑acute rehabilitation. This part‑time role offers a consistent schedule with opportunities for additional shifts and potential growth to full‑time, based on patient census and care needs.

• Provide high‑quality care to a diverse patient population, primarily long‑term care residents with some rehabilitation patients.
• Collaborate with a multidisciplinary team, including physicians, nursing staff, administrative leaders, the therapy team (PT, OT, SLP), and social workers, to support comprehensive patient care.
• Conduct patient assessments, develop care plans, and manage ongoing treatment.
• Document care using Epic EMR (training provided).
• Maintain compliance with facility protocols and regulatory standards.

  2 set days per week (Wednesday and Friday preferred).
• Shift Length: 10–8‑hour shifts.
• Coverage Area: 100‑bed facility (primarily long‑term care).
• Team Structure: 2 physicians + 1 full‑time APP (typically 2 providers onsite daily).

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Vermont.
• Experience in long‑term care, geriatrics, or rehabilitation preferred.
• Strong communication and collaboration skills.
• Ability to work independently and as part of a team.
• Geriatric experience preferred; however, new graduates are welcome to apply.

$52.33/hr – $73.27/hr
