Responsibilities- Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
- Supervise other home care staff as necessary.
- Must possess an active RN license in both VT and NH.
- ASN required, BSN or MSN preferred.
- Nights, Weekend, Holiday and on-call rotation required.
- Required: Associates or better in Nursing.
- Preferred: Bachelors or better in Nursing.
- Required: 2 years nursing
- Required: Registered Nurse - VT
- Preferred: Registered Nurse - NH
- Preferred: Registered Nurse Compact
