About the Role:We are seeking a licensed and experienced LPN to join our dedicated team of healthcare professionals. As an LPN at Allaire Health Services, you will play a vital role in delivering high-quality patient care to our residents. Your responsibilities will include:
- Providing direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
- Developing and implementing individualized care plans in collaboration with the interdisciplinary team
- Conducting assessments and reporting changes in patient condition to the nurse manager
- Assisting with treatments, procedures, and medical tests as ordered by the attending physician
- Maintaining accurate and up-to-date patient records
