The Family Practice MD/DO/APP provides the full scope of primary care, which fall under their training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance to patients.

Key Responsibilities:
  • Provides comprehensive patient care by interviewing, examining and treating clinic and/or hospital patients in order to meet their medical needs.
  • Manage long-term treatment plans for patients with chronic illness, ensuring regular follow-ups and monitoring; manage acute care and chronic medical conditions and commodities.
  • Educates patients on wellness, prevention and early detection by providing materials and resources to the patients and families.
  • Determines level of urgency of follow-up, referral, and/or consultation appointments based on examination and patient needs.
  • Prescribes medical treatment and medications to patients.
  • Ensures patient controlled-substance contracts are completed for all patients being prescribed controlled substances, monitors use of medication and documents all results.
  • Checks Vermont Prescription Monitoring System (VPMS) when appropriate.
  • Orders studies, test and ancillary services.
  • Consults with Medical Director and other professionals on staff as needed regarding patient care, assessment, and education issues.
  • Practices in a primary care setting itionally, GCHF has a Same Day Clinic, and coverage may be required at times.
  • Uses the electronic health record to document patient care and completes records according to Grace Cottage policy.
  • Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations.
  • May serve as a preceptor to medical students or other appropriate students per federal and state regulations.
  • Adheres to Grace Cottage Medical Staff Bylaws, Rules and Regulations, Standards of Behavior, policies and procedures, and Granted Privileges.
  • Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage.
  • Assists in the resolution of complaints, requests and inquiries from patients
Requirements

Qualifications:
  • Licensed to practice, or willing to license in Vermont.
  • Graduate from accredited Medical College w/medical degree & completion of a US residency program.
  • Have a record that is free from current Medicare/Medicaid sanctions and not be listed on either the OIG List of Excluded Individuals/Entities or System for Awards Management (SAM).
  • Possess a current, valid, unrestricted Drug Enforcement Administration (DEA) number if applicable.
  • Board Certified per Grace Cottage Bylaws.
  • Insurability (malpractice).
