Home Care Registered Nurse (RN) – Rutland

VNA & Hospice of the Southwest Region is looking to hire a full‑time Home Care RN in Rutland. You will provide quality home health and hospice care, supporting patients and families.

• Actively manage and assess patients’ treatment plans and needs.
• Maintain accurate, up‑to‑date documentation of nursing care.
• Ensure infection control standards and stay current on high‑tech skills (IV use, wound VAC, complex wound care).
• Conduct timely visits and educate patients and caregivers.
• Coordinate with physicians, providers, team members, and vendors to fulfill the care plan.
• Assist with patient discharges or transfers and complete necessary paperwork.

• Registered Nurse (RN) license.
• Minimum 2 years of nursing experience (preferred).
• Self‑directed, initiative‑driven, and collaborative within an agency.
• Experience with electronic medical records.
• Willingness to work a flexible schedule, including rotational weekends, holidays, and on‑call for hospice and home‑care patients.
• Exceptional bedside manner, strong communication skills, compassionate demeanor.
• Valid Vermont driver’s license and car insurance.
