Physician Assistant / Nurse Practitioner – Primary Care Join Our Team
We are seeking a compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to provide primary care in a long-term care setting and work collaboratively with an interdisciplinary healthcare team.
Key Responsibilities- Provide primary care and manage acute and chronic conditions.
- Perform patient assessments and physical examinations.
- Order and interpret labs and diagnostic tests.
- Develop treatment plans and prescribe medications.
- Educate residents and families on health and treatment plans.
- Monitor patient progress and maintain accurate documentation.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals.
- Master's degree from an accredited PA or NP program.
- Active Vermont PA or NP license
. - Active DEA registration
. - Minimum 6 months of LTC, skilled nursing, or similar experience
. - Strong clinical, communication, and assessment skills.
- Commitment to compassionate, patient-centered care.
