Position: Specialized Case Manager - Chittenden County
Location: Town of Vermont

About the Position

We’re looking for a Specialized Case Manager to join our team!

Specialized Case Managers provide intensive care coordination, assess needs, coordinate, and monitor community services and benefits for individuals over 60 or individuals under 60 with disabilities and enrolled in Choices for Care to promote health, independence, social and economic wellbeing. They serve vulnerable or at‑risk individuals and those with medical, social, or emotional challenges that add complexity to their care needs.

The role requires an understanding of community resources and includes visiting clients in the community to offer practical assistance, person‑centered guidance, and support. Particular attention is paid to the most vulnerable and at‑risk individuals in the community s is a field service position with a degree of complexity and challenge. The manager must provide their own transportation for extensive local travel and work in the office on a regular basis.

Mileage reimbursement is provided.

This position serves Chittenden County. Clients are served Monday‑through‑Friday, 8:30 am‑4:30 pm. No on‑call nights or weekends.

Requirements
  • BA/BS, preferably with a human service orientation
  • Three years of social service experience
  • Experience supporting individuals with behavioral health needs
