  Utah license required; IMLC accepted (client willing to license)

  Provider must have own workstation (client IT team supports setup)
• References: 3 professional references required post-interview
• Productivity:
  Minimum of 10 RVUs/hr

Scheduling & Shifts

• Contract Length: 26 weeks, ongoing
• Shift Length: 8-hour shifts preferred; flexible scheduling available
• Schedule:
  
  24/7 coverage need
  • Days, mids, nights, and weekends
• Minimum: 14 shifts per month (weekend shifts required)
• No call
