Transition Services Coordinator – High School Students with Disabilities (Part-Time)

Bloom Consulting is seeking a part‑time coordinator and trainer to support high school students with disabilities as they prepare for life after graduation. This flexible role blends direct instruction with coordinating services, ensuring students gain valuable skills for independence, career exploration, and future success.

• Provide one‑on‑one instruction to high school students with disabilities using Bloom’s provided curriculum (no lesson planning required)

• Coordinate and arrange services, including job shadowing, workplace readiness, and short‑term job coaching experiences (especially in summer)

• Communicate with schools, families, and businesses to schedule, arrange, and follow through on student services

• Manage the full service cycle: coordinate schedules, deliver services, and complete required documentation

• Travel within assigned counties (vehicle stipend provided)

• Bachelor’s degree preferred (education, human services, psychology, social work, or related field)

• Experience working directly with youth with disabilities (in teaching, tutoring, coaching, or related roles)

• Strong organizational and communication skills – able to balance instruction with service coordination

• Dependable, caring, and patient – able to build trust with teenagers and their families

• Reliable transportation and comfort with regional travel

• $25–$27/hour (1099 contractor)

• 10-20 hours per week, primarily during the school day

• Flexible scheduling to support both student and school needs

• Monthly travel stipend to offset costs
