About the position:
Casual shifts are available five days a week, on an as needed basis. Orientation will be with Vernon Jubilee Hospital acute care and Vernon community health services.
Social workers at Vernon Jubilee Hospital and Vernon Community Health Services play a vital role in supporting patients and families through complex health and life challenges. You’ll provide psychosocial assessments, crisis intervention, counselling, and advocacy to help individuals navigate illness, hospitalization, and recovery. Working as part of interdisciplinary teams, Interior Health Social Workers collaborate with physicians, nurses, and allied health professionals to develop patient-centred care plans that address both medical and social needs.
You’ll assist with discharge planning, connecting patients to community resources, housing supports, financial assistance programs, and home care services to promote safe transitions from hospital to home. In community health settings, social workers support individuals managing chronic health conditions, mental health concerns, substance use challenges, and significant life transitions. Their work focuses on enhancing well-being, strengthening coping strategies, and reducing barriers to care.
