JOB SUMMARYThe Home Support Worker position is responsible for providing home visits to clients as scheduled to provide companionship, home management, or other duties as indicated by the client care plan. As a Home Support Worker, you will be responsible for providing housekeeping, light cooking and transportation services to elders, persons with disabilities and others, assisting them to continue living in their homes and accessing resources in the community.
The Home Support Worker will liaise with other community services and resources to ensure the best access and utilization. The home support worker may also participate in fall prevention activities, such as shovelling walkways/packing wood. It is through the provision of these services that risk is reduced, allowing elders and other individuals the ability to remain living in their home.
Reports to Home and Community Care Supervisor.
JOB DUTIES & RESPONSIBILITIES- Participate in the planning and scheduling of client services
- Maintain effective and accurate charting for each client
- Be knowledgeable on the limits of services that can be provided based on the Adult in Home Care Program guidelines set out by INAC, client assessment, and policies of the Okanagan Indian Band
- Identify and report client concerns to HCC Nurse
- Maintain ongoing support and assistance to clients and family members
- Work effectively with each client and be able to adapt to different circumstances using tact, diplomacy and by exercising sound judgment
- Attend and participate in training programs, in-service meetings and workshops identified by the Supervisor or Nurse as required for education and service improvements
- Follow Duty to Report child protection concerns to OKIB Social Services Worker and the BC Ministry of Child and Family Services
- Commit to build and maintain trust in the client‑caregiver relationship
- Make sure client and family members are aware of the Adult in Home Care Program with activities of daily living and providing non‑medical support care services when necessary, according to service care plan
- Perform activities of daily living as outlined in each individual care plan, duties may include general housekeeping, companionship, meal preparation, carrying wood and water, SAIL exercises.
- Assist with falls prevention, such as cleaning cluttered areas, shoveling walkways, and assisting to pack wood
- Prioritize client needs and homemaking activities on an ongoing basis
- Apply problem solving and critical thinking while providing care to client, within the scope of practice, education and training
- Submit timesheets and reports as required and requested
- Perform other duties as assigned related to the position and the Adult in Home Care Program
- Performs all duties and responsibilities in accordance with the OKIB policies and procedures
- All persons employed by the Okanagan Indian Band may be required to assist the Band in providing emergency services. Duties assigned during an emergency may differ from regular duties.
