• Coordinate patient/client access and care transitions

• Follow BCCNM standards and organizational policies

Requirements:

• Graduate from an accredited Practical Nurse program

• Three years of related clinical experience

• Current BCCNM registration required

• Valid BC Driver’s License needed

• Strong skills in community and acute care nursing
